Patient Rights & Responsibilities
Patient Rights
At Luminary Health, we are committed to providing ethical, respectful, and patient centered medical care. As a patient of Luminary Health, you have the following rights:
1. Right to Respect and Dignity: You have the right to be treated with dignity, respect, and consideration at all times.
2. Right to Quality Medical Care: You have the right to receive appropriate medical care within the scope of services offered, consistent with accepted standards of practice and applicable Wisconsin law.
3. Right to Information: You have the right to receive clear explanations of your diagnosis, treatment options, risks, and benefits. You may ask questions and receive information in language you understand, including disclosure if a medication is 
compounded and not FDA-approved.
4. Right to Informed Consent: You have the right to participate in decisions regarding your care, provide informed consent before treatment, and refuse treatment to the extent permitted by law.
5. Right to Privacy and Confidentiality: You have the right to confidential treatment of your medical records in accordance with HIPAA and Wisconsin law, including secure telehealth visits.
6. Right to Access Your Medical Records: You may review and request copies of your medical records and request corrections as permitted by law.
7. Right to Know Financial Policies: As a cash-based practice, you have the right to receive clear information about fees before services are provided. Payment is due at the time of service. Luminary Health does not bill insurance.
8. Right to Continuity of Care: You have the right to receive follow-up instructions, appropriate referrals when indicated, and notification if services cannot be continued.
9. Right to Safe and Ethical Telehealth Care: You have the right to understand the limitations of telehealth and to be informed if in-person care is recommended.
10. Right to File a Complaint: You may express concerns or file a complaint without fear of retaliation.

11. Right to Emergency Care: Luminary Health does not provide emergency services. You have the right to seek emergency care by calling 911 or visiting the nearest emergency room.


Patient Responsibilities
In partnership with your healthcare provider, you have the following responsibilities:
1. Provide Accurate and Complete Information: You are responsible for providing truthful and complete medical history information, disclosing all medications and conditions, and informing your provider of changes in your health. This includes history 
of pancreatitis, thyroid cancer (including Medullary Thyroid Carcinoma), MEN 2 (Multiple Endocrine Neoplasia Type 2), pregnancy, or breastfeeding status.
2. Participate Actively in Your Care: You are responsible for asking questions, following your treatment plan, taking medications as prescribed, and attending recommended follow-up visits.
3. Use Medications Safely: You must administer medication as instructed, store medications properly, not share prescriptions, and report side effects promptly.
4. Understand the Scope of Telehealth: You are responsible for ensuring you are located in a state where your provider is licensed at the time of the visit and for seeking emergency care when necessary.
5. Financial Responsibility: Payment is due at the time of service. Appointments must be canceled or rescheduled at least 48 hours in advance. Visits canceled less than 48-hours prior to a scheduled appointment will result in forfeiture of fees as outlined in the Notice 
of Financial Policies. There are no refunds for services rendered. Medication costs are non-refundable.
6. Maintain Respectful Conduct: Patients must treat staff and providers respectfully. Luminary Health reserves the right to terminate the provider-patient relationship for inappropriate conduct, medication misuse, or noncompliance.
7. Follow-Up and Continuity of Care: You are responsible for completing recommended lab work, attending follow-up appointments, and notifying Luminary Health if you begin care with another provider for weight loss medications.
8. Understanding Treatment Limitations: Individual results vary. Outcomes cannot be guaranteed. Compounded medications are not FDA-approved and carry unique risks.​
